Billing Procedures     After a thorough exam and x-rays, we develop a treatment plan. The treatment plan states necessary and optional treatment needs. This allows us to prioritize treatment sequence and estimate total fees and insurance benefits. At the next appointment Dr. Kanda discusses the treatment plan. Patients are encouraged to ask questions about suggested treatment and options.    The total treatment plan can be submitted to your insurance carrier for a pre-estimate of benefits. We ask that you pay your co-pay at the time of service. If this obligation cannot be met, we can make other financial arrangements.     5% bookkeeping courtesy for cash payment of total at time of treatment.
